Transfer Medical Records

The care our doctors provide is improved when they have information about your past medical history. We recommend you have your medical records with you at the time of your first appointment. You can request your records by calling your previous doctor. If you are unable to obtain your records, our staff will assist you in completing paperwork to request records be sent to our office.
